Cancellations

Payments

Primary Care Provider Relationship

Prescription Services

- You understand that your health care information may be shared with other individuals for scheduling and billing purposes.
- You understand this is a telehealth-based service that is not equipped to handle medical emergencies. IF YOU HAVE AN EMERGENCY THAT NEEDS IMMEDIATE RESPONSE, YOU AGREE TO CALL 911 OR GO TO YOUR NEAREST EMERGENCY ROOM.
- You give your informed consent to telehealth as an acceptable form of delivering health care services to you, and that this consent will cover any and all Services provided to you through the Website.
- You may withdraw your informed consent at any time by discontinuing services through the Website or by deactivating your account, if you have one.

You expressly consent to receive recurring automated promotional, transactional, and personalized marketing text messages (e.g., SMS, MMS, RCS) from Revive and our service providers sending these messages on our behalf. These messages may be sent using an automatic telephone dialing system, AI-assisted technology, or other automated systems to the mobile number you provide at sign-up or anytime thereafter. You agree that you consent to receive messages regardless of any state or federal time-of-day restrictions or the registration of your number on any Do Not Call list.

If you intend to stop using the mobile number you used to subscribe to the Program, you agree to complete the opt-out process set forth above before ending your use of the number. YOU AGREE TO INDEMNIFY, DEFEND, AND HOLD REVIVE HARMLESS FROM ANY CLAIM OR LIABILITY RESULTING FROM YOUR FAILURE TO NOTIFY US OF A CHANGE IN YOUR INFORMATION, INCLUDING ANY CLAIMS UNDER THE TELEPHONE CONSUMER PROTECTION ACT (47 U.S.C. ยง 227) OR SIMILAR STATE AND FEDERAL LAWS.
